Native Diabetes Prevention Intervention Conference and Native Fitness Training
REGISTRATION
Diabetes Conference Costs and CEUs
The regular registration fee for the conference is $320 USD per person if registering by January 16, 2009. Participants registering after January 16 or on-site will be charged $420 USD. The registration fee covers all conference activities and materials, lunch on Monday, Tuesday, and Wednesday, as well as a Certificate of Participation and Completion. All participants will receive 2.1 Continuing Education Units from the University of Oklahoma.
Native Fitness Training for Special Populations Costs and CEUs
The regular registration fee for the training is $290 USD per person if registering by January 16, 2009. Participants registering after January 16 or on-site will be charged $390 USD. The registration fee covers training manual, Native Fitness T-shirt, Native Fitness Music CD, and 1.8 Continuing Education Units from the University of Oklahoma.
Registration Fee to attend both the Diabetes Conference and the Fitness Training will be offered at $525 USD. A savings of $85!
PAYMENT AND CANCELLATION POLICY: *
Payment must be received five days before the conference or paid on-site. Purchase orders can be accepted only if payment will be made within 30 days of the conference. To avoid cancellation fees, please notify the OUTREACH Registration office (mail or fax) at least one week prior to the conference. A $50 cancellation fee will be charged if written notification is received less than one week prior to the conference.
REGISTERED ‘NO SHOWS’ WILL BE CHARGED $100! *
